So I bought a 998 last year that I am rehabilitating.
What is this gear in-line with the speedometer cable?
the short end attaches to the speedometer.

Must be some kind of "speedo healer" mine doesn't have one.

Looks like it, possibly slows down the gearing so that a kmh speedo will show mph?

Although I doubt it would cost much less than just changing the speedo?

It’s a mechanical ratio adapter. Used on cars in the day when all the speedometers were cable driven. Use when changing wheel size or diff ratios. Why on earth it would be on a 998 is anyone’s guess. Maybe has a duff speedo.
